If you are interested in scheduling a consultation with Lauren, please completely fill out the appropriate set of forms – which you can download below. Return the completed forms to us as instructed on the bottom of the first page. Once we receive your completed forms, you will receive a phone call to discuss scheduling your consultation.

Please note, all consultations are virtual via a HIPAA secure tele-medicine platform. No in-person appointments are available at this time.

Treating a hormone imbalance can be complicated – and allowing the time necessary to educate, discuss, and mutually formulate the best individual plan of care for you is our goal. We want to give you the best possible care you deserve. Unfortunately, our current “for profit” insurance industry does not prioritize, nor often reimburse for this time spent.

We have attempted to make our fee schedule reasonable and easy for you. Click the button below to view our current Fee Schedule. Pricing is based on time spent and complexity of each individual appointment. Payment is required in full at the time of scheduling and/or service. We accept VISA, Mastercard, American Express, Discover Cards, and select HSA/FSA cards.
